Site member O5-14 (other username taken from their old sandbox page) recently posted two "test" pages to the mainsite, which essentially were site member lists for the mainsite (from what I remember):

Near-identical text on the front page, minus the "contact these people if your sandbox is vandalized" bit. Someone has sent them a PM to change their username, but given that their test pages were posted an hour apart, keep an eye on.

Given that their current account doesn't have much site history except breaking the rules, I say tell them to create a new account. Letting a disallowed username to stay because they weren't careful just invites more people to subvert that rule.

Also, if they can't change their name anymore, great, a simple banning of O5-14 will prevent them from mucking up the main site.
