Particularly of note is the section, "Let's be more descriptive about the incident logs. Every log should have in it the date and time the incident happened, where in the facility it took place, and who was involved with it. We should also describe what occurred directly after the incident occurred, to include if they changed any procedures or did any direct assessments. Let's not forget to note if there is any follow-up monitoring or research that comes from it."
